All participants should turn in all the required documents on Monday, April 27th 2015 before 6 p.m. at ITS International Office (Rectorate Building 2nd Floor). The required documents are :

  1. Online registration form
  2. Recommendation letter from HMJ/BEM-F/BEM-ITS/LMB ITS
  3. Statement letter of commitment to actively contribute to ORMAWA in ITS (Period: 2015-2016. Include participant's signature and witness' signature)
  4. Transcript (in Bahasa Indonesia is acceptable)
  5. Photo 3×4 (colored, 2 pieces)
  6. Copy of certificates (max. 3)

All participants should attend the briefing for AKL which will be held on :

Time       : 6.30 p.m.

Venue     : Room D-303, Mechanical Engineering Dept.
